AGREEMENT
FOR SERVICE
I/WE the undersigned, by submitting this online form, herewith request
TNS Answering Service to provide service as noted hereto
subject to the terms and conditions as provided hereunder.
TERMS AND CONDITIONS
The listed applicant agrees to to pay all charges and fees hereto noted
by TNS Answering Service, it being clearly understood all
such charges and fees for service(s) by TNS Answering Service
are PAYABLE IN ADVANCE. TNS Answering Service reserves the
right to cancel or discontinue service if payment for such service is
not received within five (5) days of due date and further to have disconnected
such line or telephone equipment from its premises and used by the applicant
is payment is not received within fifteen (15) days of due date. TNS
Answering Service is further absolved of any recourse by the applicant
for such discontinuation of service, attestation of such, being the
online submission of this form.
Cancellation of service may be made by either party upon 30 days written
notice.
TNS Answering Service agrees to maintain its facilities
open for service on a 24 hour basis including Sundays and Statutory
Holidays and intends to employ only diligent, careful and trustworthy
employees. TNS Answering Service further intends to maintain
close supervision over its employees and their work so as to eliminate
all possible error. Nevertheless, in view of the nature of the business
of TNS Answering Service, TNS Answering Service
will not accept the responsibility or liability of any loss, damage
or inconvenience caused the applicant by any errors or omissions on
the part of TNS Answering Service's employees. TNS
Answering Service is hereby further absolved from any loss, damage
or inconvenience caused by any failure of any telephone equipment owned
or operated by Telus. TNS Answering Service will maintain
vigilant watch for any technical failures and will report such failures
without prior advice to the applicant to Telus.
TNS Answering Service agrees to receive and record any
incoming messages on behalf of the applicant and deliver same in an
agreed format. TNS Answering Service will not accept incoming
Long Distance Calls, Telegrams or Cables on behalf of the applicant
without indication to do so below. This request becomes a binding
contract when the applicant's telephone circuit has been made with
TNS Answering Service's equipment and shall remain in force
until notice of cancellation is made in writing. The Subscriber agrees
to provide in writing an
outline of his business or profession, list of key personnel and their
respective positions and duties also business and home addresses and
phone numbers.
